VitalGuard Subscription Services Cancellation and Refund Policy

⚠️ IMPORTANT NOTICE: SCOPE OF POLICY & INSURANCE COVERAGE

Core Remote Patient Monitoring (RPM) Services: VitalGuard’s core RPM services, monitoring equipment, and clinical escalation tracking are typically covered by Medicare, Medicaid, and private commercial insurance plans. This Subscription Services Cancellation and Refund Policy DOES NOT apply to any services billed directly to your insurance provider.

Applicability: This policy applies solely and exclusively to voluntary, out-of-pocket, or cash-pay optional subscription programs (the “Subscription Services”) offered by VitalGuard in partnership with our network providers (e.g., specialized wellness tracks or non-reimbursed clinical additions). If you do not participate in a cash-pay subscription plan, this policy does not affect your account.

Subscription Services

  • Your membership in an optional subscription program (“Subscription Service”), including your ability to access these specialized services, begins when you have completed the intake form, at which time you will be charged for the first month of the Subscription Services.

  • The Subscription Services include access to providers, medications where medically appropriate, and online support.

  • Please note if you are disqualified by our medical provider for any medical reason, you will receive a full refund for that billing cycle.

  • There is no guarantee of results or effectiveness due to your participation in the Subscription Services.

Cancellation Policy

  • You may cancel your optional Subscription Service at any time for any reason. To cancel your Subscription Services, please contact customer service at support@myvitalguard.com and request a cancellation, or utilize the self-service cancellation process outlined within your authenticated online dashboard account.

  • To ensure a smooth process, we require your cancellation request to be received at least seventy-two (72) hours prior to your upcoming billing date. If this notice window is not met, your subscription payment method will be charged on the next scheduled billing date, and the cancellation will take effect on the subsequent billing cycle.

  • After cancellation, you will continue to receive access to the Subscription Services through the end of your current active billing cycle.

  • Please see our Refund Policy below to determine if you are eligible for a refund of your canceled Subscription Service.

Refund Policy

  • If your Subscription Service is cancelled due to medical disqualification by our affiliated network medical provider, you will be issued a refund for the remainder of your Subscription Service charges following the date of disqualification.

  • Other than cancellation due to direct medical disqualification, IN NO EVENT SHALL YOU BE ISSUED A REFUND UPON CANCELLATION OF THE SUBSCRIPTION SERVICES.

  • Prescription Medication Rules: Federal law generally prohibits the return of prescription medications to pharmacies for refund or reuse once they have been ordered and dispensed to a patient, in order to protect public health and safety. This policy ensures that all medications provided are handled and dispensed with utmost care and compliance. If approved by administrative review, refunds will only be issued for your most recent billing cycle and will not be issued for past, historical billing cycles.

  • Damaged or Incorrect Items: Please inspect your medication immediately upon physical receipt. If the medication appears to be damaged or incorrect, please contact VitalGuard customer service immediately.

    • Damaged Items: Damaged items may be replaced by the dispensing pharmacy partner upon receipt of physical or photographic evidence of damage.

    • Incorrect Items: Incorrect medications will be replaced directly by the partner pharmacy.

Subscription Services FAQs

What is included in my Subscription Services price?

Your Subscription Services fee represents a single, transparent price for the out-of-pocket subscription purchased. This may include consultations (both live and via electronic form submissions and reviews) with a clinical Provider for medications that are fulfilled through a partner pharmacy or other health care services on which we collect payments, lab services, care support, our 24/7 patient support line, and other specialized services. Services, labs, and medications covered within a single Subscription Service price may vary based on your selected tier.

What if my provider determines I am no longer eligible for the Subscription Services?

If your affiliated Provider determines you are no longer medically eligible for the Subscription Services, your optional subscription will be canceled, and a prorated refund may be issued. If you have questions or concerns about an eligibility-initiated cancellation, please contact our customer service team at support@myvitalguard.com.

Legal Governing Terms

Unless otherwise stated herein, the main VitalGuard Terms and Conditions and Notice of Privacy Practices (NPP) govern this Subscription Services Cancellation and Refund Policy.

In all events, you remain responsible for all fees from core clinical services and healthcare services rendered to you; this specific Subscription Services Cancellation and Refund Policy strictly governs commercial fees for optional out-of-pocket Subscription Services only.
