VitalGuard LLC (“VitalGuard”) utilizes advanced digital health technologies, Remote Patient Monitoring (RPM) infrastructures, and interactive telecommunications modalities to facilitate clinical care, physiological data review, and clinical escalation tracking at a distance.
To provide you with seamless access to licensed healthcare professionals authorized to practice medicine and prescribe medications via asynchronous or synchronous platforms, VitalGuard partners with dedicated professional medical networks and clinical groups.
1. Nature of Telehealth Services
Telehealth involves the delivery of healthcare services using electronic communications, information technology, and digital data transmissions between a patient and a clinician who are not in the same physical location. These services may include, but are not limited to, remote physiological monitoring, online medical consultations, digital health questionnaires, asynchronous chart reviews, and interactive audio/video conferencing.
2. Expected Benefits & Limitations
- Benefits: Improved access to specialized clinical professionals, efficient tracking of daily physiological biomarkers, reduced logistical burdens associated with travel to brick-and-mortar clinics, and accelerated clinical evaluation cycles.
- Limitations: A telehealth evaluation does not allow for a comprehensive in-person physical assessment. In rare instances, technological failures or poor internet connectivity may delay or distort data transmissions, requiring an alternative follow-up method.
3. Privacy & Security Safeguards
VitalGuard employs strict administrative, physical, and technical safeguards designed to preserve the absolute confidentiality and security of your Protected Health Information (PHI). In strict accordance with the Health Insurance Portability and Accountability Act (HIPAA), all data transmitted through our platform—including biometric markers, messaging logs, and clinical intake profiles—is fully encrypted at rest using AES-256 protocols and in transit utilizing TLS 1.3 cryptographic pipelines.
4. Patient Rights & Voluntary Participation
Your consent to receive healthcare services via telehealth is entirely voluntary. You maintain the absolute right to withhold or completely withdraw your consent to telehealth services at any time throughout your care journey without affecting your right to receive future standard in-person care or treatment from alternative healthcare providers.
5. Acknowledgment & Electronic Signature
By checking the consent boxes during your digital onboarding process, clicking “I Agree,” or continuing to utilize the VitalGuard remote monitoring platform, you formally acknowledge that you have read, understood, and accept all risk disclosures, operational boundaries, and partner medical group policies outlined within this Telehealth Informed Consent document.
